SAFE Proposal Seeks Shared Playbook for Agentic AI Security Incidents

Agentic artificial intelligence systems do more than generate answers. They can invoke tools, access enterprise applications, interact with data, use credentials, and take actions across business workflows.

That expanded ability also creates a difficult security question: what happens when an AI agent behaves unexpectedly, circumvents a control, exposes sensitive information, or becomes part of a cyberattack?

In most cases, the affected organisation investigates the incident internally. The technical findings, control failures, and operational lessons may never reach other companies deploying similar systems.

The Linux Foundation and members of the Open Secure AI Alliance now want to create a mechanism for changing that.

On August 4, 2026, participants in the alliance published a Request for Comments (RFC) for the Shared AI Findings Exchange (SAFE) Working Group. The draft proposes a community framework through which organisations could confidentially report and analyse AI-security incidents and near misses.
